Program Acknowledgment, Consent, and Release
Dear Children, Youth, and Parents/Guardians:
Parents/Guardians with children participating in the Sound Space program must return a signed “Program Acknowledgment, Consent, and Release.” You agree that your acceptance within the linked registration form is equivalent to a legal signature. The form has five main parts, discussed below:
- Sound Space’s Relationship to Trinity UMC.
Sound Space is a program of Trinity United Methodist Church. - Work Product Acknowledgment and Assignment
This part of the form gives Sound Space rights to use music and audio projects or other work that is created during a Sound Space program (called “Product”). Certain Products may be selected by Sound Space to be performed, recorded or broadcast, in part or full, and the proceeds will benefit future Sound Space programs. To the extent a teen, volunteer, or staff member helps create Product that is selected to be performed, recorded, sold, or broadcast, the creator retains copyright. - Photo, Video, and Media Consent and Release
This part of the form gives Sound Space (and any media source contacted by Sound Space) permission to use a teen’s name, photograph, or video in connection with a Sound Space program. - Miscellaneous
This part, in part:
– gives Sound Space permission to share data with other groups who need it to implement and evaluate Sound Space programs and to find other opportunities for Sound Space teens; and
– states that Sound Space and its partners are not responsible for any property that is lost or stolen. - Program Rules
We include program rules and consequences so that children have a clear understanding of Sound Space expectations.

Sound Space’s Relationship to Trinity UMC
I understand Sound Space is a non-sectarian, non-profit program of Trinity United Methodist Church. Trinity provides practice and performance spaces as well as material support to Sound Space. Trinity may reference Sound Space in internal and external communications.
Work Product Acknowledgment and Assignment
With respect to any product that my child creates or contributes to as a participant in a Sound Space program, including original musical scores, podcasts, audio recordings, documents, videos, tapes, photographs, records, instruments, drawings, paintings, sculpture, other artwork, or similar product (“Product”), I assign to Sound Space permission to continue use and performance as part of this program.
Permission is also rescinded if the Sound Space program ceases to operate for two continuous calendar years.
No Product will be deemed or construed to be a “work made for hire” within the meaning of that term under the federal Copyright Act. I also understand that I will retain Sound Space retains copyright of the Products my child creates or contributes to, such as a musical performance. I acknowledge the intent of Sound Space is to allow the re-broadcast and distribution of performances and podcasts that may include my child.
Photo, Video, and Media Consent and Release
I consent to the use by each Participating Entity of my child’s name, image, and/or likeness in any audio, visual, or written media relating to a Sound Space program (“Media”). I further grant to each Participating Entity the right to use, exhibit, copy, broadcast, perform, reproduce, modify, and display the Media, in any and all formats, for demonstration, illustration, promotion, exhibition, or editorial use, and to use my name in connection with the Media if a Participating Entity so chooses. Permission is rescinded if the Sound Space program ceases to operate for two continuous calendar years.
Miscellaneous
I understand that certain information provided may be shared with other entities for purposes of administering programs and research and obtaining funding. I consent to the release of such information to such entities and possible donors. I agree that the Participating Entities are not responsible for lost, stolen, or damaged property, including articles of clothing. Sound Space may sell, assign, license, or otherwise transfer all rights granted to it by this agreement.
Program Rules
I understand the Sound Space rules and consequences and will share them with my child.
Rules
- Come prepared to participate.
- Allow the leaders to help and teach, and allow others to learn.
- Follow leaders’ directions promptly.
- Keep hands and feet to yourself.
- Always be respectful of and kind to others and their property.
Consequences for Not Following Rules
- Warning (name on leader’s notepad)
- Sit by self out of activity (5 minutes)
- Twice out in one day means a call to parent/guardian.
- Two weeks in a row, sitting out means out of the program.
